Download the forms.

The printable application may still list the regular price — if so, just write in the current special price. One or both parents must sign the minor waiver forms. Prefer paper? We have forms at sign-up, or we can email them to you.

Mail completed forms per the instructions on the application: CCDR, c/o Bob White, Treasurer, 11865 Roxbury Rd, Charles City, VA 23030. If your forms are in good order and filled out completely, you can pick up your membership stickers on your first visit.
